If you have a medical problem with your eyes such as glaucoma, cataracts, diabetes, injuries, infections, allergies, dry eye disease or any other medical issue, your medical insurance benefits can be used for your visit to our office.
Routine Vision Coverage
Some individuals have “routine vision insurance” through their employer. Some plans pay toward the optometric eye exam only, others will sometimes pay towards the contact fitting fee, and some even pay toward materials.
If you are uncertain about your benefit eligibility, you will need to check with your human resource department or call your insurance company directly and inquire about eligibility.
Questions you should ask:
* Does my insurance plan provide vision benefits and coverage?
* Can I see the eye doctor of my choice?
* What does my plan allow?
To verify your vision coverage, we will need a phone number for verification of benefits, a group, plan or policy number, the social security number of the insured, member and patient’s date of birth, and name of employer. Verification of insurance benefits must take place 24 hours prior to your appointment.
When you arrive for your appointment we will need a copy of your medical and vision insurance plan policy cards. You will be expected to pay in full the required copay or the difference in the amount allowed at time of service. If for any reason the insurance company pays less than quoted you will be responsible for any amount unpaid.
We participate with most major medical and vision insurance plans and will file claims to your insurance carrier on your behalf. You can click on your insurance below to check your benefits before your visit with us. If your insurance is not listed below, or if there is not a link available, you can go to the website listed on your insurance card to check your benefits and to make sure Hometown Vision Care is in network with your carrier. If Hometown Vision Care is not in network with your carrier, we will provide you with all necessary documentation to file your claim.
